What is your presentation about?
Although at high risk, the majority of people living with type 2 diabetes and cardiovascular or chronic kidney disease are not receiving optimal guideline effective medical therapy. This is in part due to traditional care models being fragmented and siloed. Changing this paradigm will require a new, innovative approach with care delivery models that prioritize adoption of efficacious therapies and can ultimately improve patient outcomes and quality of life. The Cardiometabolic Center Alliance has developed a team based, coordinated, comprehensive model of care delivery that has been successfully implemented across multiple health care organizations as evidenced by longitudinal patient registry data.
What makes this topic important in 2023?
Suboptimal use of guideline directed medical therapy in this multimorbid, high risk, growing patient population remains despite the abundance of scientific data and professional society recommendations. Financial and societal impacts will continue to place a tremendous burden on health care systems as well as those living with type 2 diabetes until care delivery improves in this area.
How did you become involved with this area of diabetes research or care?
I have been involved in clinical research, including clinical trials involving patients with cardiometabolic disease for close to 20 years. Developing new, efficacious treatments for this patient population has been my passion and somewhat of a personal mission as several of my family members have been impacted by diabetes and related cardiac and kidney complications.
How do you think your presentation will impact diabetes research or care?
Data from the Cardiometabolic Center Alliance multicenter patient registry demonstrates that improvements in care are possible when patient care is delivered in a collaborative, team-based, coordinated manner. This model has been successfully replicated across multiple organizations nationally. The resulting improvements seen in patient outcomes with the effective use of guideline directed medical therapy should prove as a catalyst to transform care delivery in cardiometabolic disease on a large scale.
What are you most looking forward to at the 83rd Scientific Sessions?
The opportunity to connect face to face with peers from around the country and the world who are similarly passionate about improving the lives of those living with diabetes.
