Improving Diabetes Management in the Elderly—DSMES in Home Health

What is your presentation about?
This presentation focuses on bringing DSMES into the home setting for the older adult population who are homebound. We initiated a program through our home health services to improve the outcomes of people with diabetes. We have seen great results in outcomes, and our patients are now able to self-manage their diabetes in the comfort of their homes.
How do you hope your presentation will impact diabetes research or care?
By demonstrating alternative methods to deliver DSMES specifically in the home setting, we can assist older adult patients who are unable to leave their homes to attend DSMES in an outpatient setting. Our goal is to further evaluate how using DSMES and the seven self-care behaviors can help patients of any age to self-manage their disease condition.
How did you become involved with this area of diabetes research or care?
I had the privilege of working with Barbara Eichorst, the current Vice President of Health Programs at the ADA. She initiated our Daily Difference with Diabetes program at CenterWell Home Health to provide evidence-based diabetes care in the home setting.

This presentation outlines the key steps taken to deliver specialized diabetes education in the home health setting. We’ll delve into the strategic rollout plan across multiple locations, recap the essential resources utilized, and discuss ongoing actions and outcome tracking.
How do you hope your presentation will impact diabetes research or care?
My aim is to raise awareness about the significant impact of Diabetes Self-Management Education and Support (DSMES) in home health. This represents new innovation in the delivery of DSMES.
How did you become involved with this area of diabetes research or care?
My role as DCO for advanced clinical programs in my region allowed me to participate in the development and planning for our Daily Difference with Diabetes program. I was immediately drawn to the influence that a program like this can have for people that are homebound and have skilled needs that often include many comorbidities.
