Pro-Islet Amyloid Polypeptide Processing in Diabetes

Islet amyloid polypeptide (IAPP or amylin) is a peptide produced and secreted by pancreatic beta cells along with insulin, and just as insulin is processed from a larger precursor form (proinsulin), IAPP is processed from a larger precursor (proIAPP). We have developed assays that allow us to measure incompletely processed forms of proIAPP in human plasma and have found that there is disproportionately high levels of a partially processed proIAPP intermediate form (proIAPP1-48) in the fasting plasma of humans with type 1, but not type 2, diabetes. In this presentation I describe how proIAPP forms may be novel biomarkers of beta cell function in diabetes and present some of our work aiming to understand the mechanism that leads to persistent proIAPP secretion in type 1 diabetes.
